Referral Requirement under UC SHIP (Anthem Blue Cross)
-
In order to access specialty care, you must seek a referral from Carbon Health Primary Care services. If a referral is not obtained in advance, the claim will be denied.
-
Please visit Carbon Health’s website or download their app to book an appointment.
-
Please note: If you are on an HMO insurance plan, Primary Care services outside of the basic covered services will be provided through your HMO-designated primary care provider rather than Carbon Health.
Referral Requirement under Medi-Cal or Other Private HMO Insurance Plans
If you waived UC SHIP and subscribe to Medi-Cal or a private HMO insurance plan, you are still entitled to access Carbon Health’s services. However, your Medi-Cal or HMO insurance plan controls your designated primary care benefits, not Carbon Health.
